Mercerville Family Dental – Hamilton Township NJ

Mercerville Dental First Visit

At your initial appointment, Dr. Loutfi, Dr. Stern, Dr. Popkin or Dr. Germani will do a thorough evaluation and examination, as well as take radiographs if any are needed.  During your visit, the doctor will consult with you on your dental condition and recommend treatment options if needed.    Occasionally, simple procedures can be performed on the same day as the consultation. However, a complex medical history or treatment plan will require follow-up appointment(s) to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any x-rays if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • You can also bring with you, the patient registration form you can download, print and fill out online through this website.

X-Rays

If your previous dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, obtaining them from the dentist in person may help expedite the process, and you can bring them with you on your first visit.   If additional films are necessary, they can be taken at our facility.
